Fill in the employee, date, location, what happened in plain English, body part affected, source object, and pick a classification: First aid / Near miss — not recordable. Still logged for learning. Medical — beyond first aid (sutures, prescription meds). Recordable. Restricted work — they came back but couldn't do their normal job. Recordable. Days away — missed work days. Recordable. Fatality — also report to OSHA within 8 hours. Recordable cases auto-assign a sequential OSHA case number per year. Sign-off (your name + date) locks the narrative — required by 29 CFR 1904.32 for the annual summary. Witnesses Open the incident → + Add witness . Name, role, phone, statement, and a drawn signature. A signed witness statement carries way more weight with carriers and in litigation than an unsigned one. Signatures get embedded in the 301 report and workers' comp packet PDFs. One PDF containing: a cover sheet with claim summary + your insurer info, the 301-style incident detail with witnesses and their signatures, and up to 10 incident photos embedded full-page. You're exempt from recordkeeping if you had 10 or fewer employees at all times during the prior year, OR your NAICS is on OSHA's partial-exempt list (most retail, services). You're required if you had 11+ employees AND you're in a covered NAICS (construction trades, landscaping, cleaning, pest control, manufacturing).
